Event Request Form

Your request must be submitted at least 30 days prior to your event.
If your event must be rescheduled or cancelled, please notify the church office 24 hours prior to your event at Martha Holcomb or call 972-625-1281.

All fields are required unless otherwise noted. If event is for multiple weeks, only enter information once and note the dates for each week.
